New Patient Packet

Your child's immunization record should be brought to the nurse at the time of registration. Please submit any medical exemptions to the nurse every year. New students are required to have a physical completed within the 12 months prior to their enrollment date. Please submit your child's physical no later than 30 days after enrollment. If no physical has been received, your child will be scheduled to see the school doctor.

Effective July 2018, all students in Pre-K, Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th grade will also be required to have a physical. If you do not want the school to complete the physical, please notify the Health Office every year.
