1) Please click the link below to access the REQUIRED health form, athlete/parent contract, and concussion policy sign-off. These forms MUST be read, signed, and return to the Athletic Director or School Nurse before the student is able to participate in a school sport.

2) Additionally, students will need to complete the green "Sports Eligibility Card." This can be obtained from the Athletic Director or School Nurse. Please email the Athletic Director at jarnold@broctoncsd.org to set up a time to pick up a sports eligibility card. 

3) Lastly, please be sure your child's imPact test and physical are up-to-date. Physicals are good for one calendar year, while the ImPact test is good for two years, unless you child has sustained a concussion, in which a post-injury test must be taken. Check with the School Nurse to see if your child needs either of these updated.
